Instructions MUST be read before downloading Mountain Waters Release Form:

  • This liability release is for rafting only. Additional releases for zipline trips, rentals or shuttles will be done on the day of your trip.
  • Please do not send forms to Mountain Waters Rafting. Turn them into your group leader or bring them with you when you check in for your trip.
  • Colorado State Law requires the signature of participants under 18 along with a parent or court appointed legal guardian on this release.
  • Each participant is required to have his or her own form; siblings, etc can not be listed on the same form.
  • By clicking on the link below you are attesting to being the parent or court appointed guardian of the minor you are signing for. Misrepresentation is fraud.
